Welcome to the PeakTPA provider portal for medical and institutional providers who bill the following health plans:


  • ArchCare
  • CHS Buffalo
  • Complete Senior Care
  • Elder One

Already have an account?  Enter the username and password you selected when registering and click Login in the box to the right to enter the portal.


REQUEST ACCESS

To request access for one of the health plans above, send email to providerportal@careventionhc.com with the following information:


 - Health plan you bill for services from the above list

 - Your first and last name

 - Email address to be registered to your account 

 - Your phone number

 - Your provider, medical group, or facility name

 - TaxID

 - Your NPI (if billing for a single provider include the individual provider ID, if billing for a medical group include the group NPI, if billing for a facility include the organization's NPI)


You will receive credentials from providerportal@careventionhc.com once your account has been activated. Please allow three (3) business days for response to your request.  
 
 
 
 
SMART DATA STREAM PORTAL

Peak has partnered with Smart Data Solutions to provide access to the Smart Data Stream Provider Portal for direct submitters!  You can upload a claim file if you're able to create an 837, or you can do direct data entry and key in a new claim.

Institutional Billers

The National Uniform Billing Committee (NUBC) was developed to maintain a single billing form and standard data sets to be used nationwide by institutional, private and public providers and payers for handling health care claims.
